Description of the Puppy:

The puppy, a Labrador Retriever mix, age approximately 8 weeks, identified by microchip number 123456789, currently in good health, and vaccinated as per shelter records.

Adoption Terms:

The adopter agrees to provide proper care, nutrition, and medical attention for the puppy. The adoption fee of $200 is payable upon signing this agreement. The adopter commits to bringing the puppy for regular veterinary care and immediate attention if health concerns arise.

Responsibilities of the Parties:

The shelter ensures that the puppy is in good health at the time of transfer. The adopter agrees to keep the puppy indoors, provide necessary vaccinations, and socialize it appropriately. Both parties agree to communicate and cooperate to ensure the well-being of the puppy.
