Sample Utah Last Will and Testament

Parties:

Testator: Alice Johnson
Address: 789 Maple Street, Salt Lake City, UT 84111

Executor: Robert Williams
Address: 456 Oak Avenue, Salt Lake City, UT 84112

Declaration:

I, Alice Johnson, being of sound mind and legal age, declare this to be my Last Will and Testament. I revoke all prior wills and codicils made by me.

Specific Bequests:

I give and bequeath the following items:

• To my daughter, Emily Johnson, my residence located at 789 Maple Street, Salt Lake City, UT.
• To my nephew, Michael Lee, $5,000 in cash from my bank account at XYZ Bank.

Residuary Estate:

All the rest, residue, and remainder of my estate, of whatever kind and wherever situated, I give, devise, and bequeath to my son, David Johnson.

Executor:

I appoint Robert Williams as the Executor of this will. If Robert Williams is unable or unwilling to serve, I appoint Susan Davis as successor Executor.
